Mandatory Vaccine Receipt Submission
As we know, OWHA is mandating that all players, volunteers, bench staff, on- and off- ice officials MUST provide proof of vaccination to continue participating in sanctioned activities.
Click here to access the Vaccine Submission Form and choose either the Player form, or the Staff/Other form if you are not a player.
You will be required to fill in the date of first dose, date of second dose as well as upload your vaccine receipts for both doses. This will be stored digitally that only a select few from the association that will have access to. OWHA has reserved the right to complete random audits with associations to ensure they comply of this.

Also as stated in the information from OWHA, you must have received your second dose by Oct 17,2021, to continue playing/assisting/volunteering/officiating etc past the November 1, 2021 deadline.
Everyone needs to have the form filled out by Oct 17, 2021 in order to continue participating in any ice time or event etc.
We urge everyone to get the form filled out asap to give us time to ensure everyone is complete and no one misses out on any ice time/events.
